CISD Athletic Physicals

lease see the opportunity for Sports Physicals at CISD. Paperwork MUST be completed in order to get a physical on site. Please complete the Google Form Registration if interested in utilizing this opportunity.
As a reminder: Incoming 7th, 9th, and 11th grade students involved in UIL (athletics, band, cheer, honeybees) MUST have an updated physical on file.

Thursday, May 2 at CJH from 9am-3pm
Tuesday, May 7 at CHS from 9am-3pm
